Bone alkaline phosphatase (BAP) is the bone-specific isoform of the enzyme serum alkaline phosphatase. It is a glycoprotein (a molecule consisting of a carbohydrate and a protein) found on the surface of osteoblasts and echoes the activity of these cells in bone metabolism. BAP is generally considered to be a sensitive and reliable indicator of bone metabolism. During the bone remodeling process, if the resorption or the bone degradation rate is greater than the rate of bone formation, the resulting bone loss can lead to metabolic bone disease (osteoporosis, Paget disease) and higher susceptibility to fractures. Osteoporosis is a bone disease that causes a loss of bone density, making bones weak and brittle that increases the risk of fractures. Paget disease on the other hand is the chronic bone disorder of the bone remodeling process, in which the body absorbs an old bone and forms an abnormal new bone. The bone-specific alkaline phosphatase blood test (BAP) is used to measure the levels of bone alkaline phosphatase (BAP) in the blood, the elevated levels of which may indicate metabolic bone diseases including Paget disease, osteomalacia, and other states of high bone turnover. It may also be used to monitor the effectiveness of antiresorptive therapies including postmenopausal osteoporosis treatment.
